Artificial intelligence (AI) is rapidly becoming part of our lives including organizing calendars, writing emails, summarizing information, and even generating workout plans or recipes. Because AI can be friendly, empathetic, and available day and night, it’s no surprise that more people are turning to AI for emotional support, and in some cases, for support resembling psychotherapy.
The appeal makes sense. After a few clicks of the keyboard, an instant answer appears. It doesn’t judge, so there’s no shame. If there’s a question at 3:00 am, there’ll be an answer at 3:00 am. Also, it’s free.
But when it comes to mental health treatment, convenience and competence are not the same thing.
It’s critical for people to consider the risks of relying on AI as a substitute for psychotherapy because it can (and likely will) do more harm than good.
Here are a few (but not all) of the dangers of using AI for mental health advice:
False or biased information
One of the greatest dangers of using AI for psychotherapy is that AI can confidently share inaccurate, incomplete, false, or biased information. Unlike a licensed mental health professional, AI doesn’t have training, supervision, ethics, and clinical judgment the way that a professional does. AI is a computer; it generates output based on patterns in data of input. Sometimes, the sources it uses haven’t been updated. It can also misinterpret sources. That means it can sometimes:
- Provide outdated or oversimplified mental health information that is not applicable to specific individuals
- Misinterpret symptoms or confuse one condition for another because mental health diagnoses have many overlapping symptoms
- Miss important differential diagnostic considerations found in medical, developmental, cultural, or other factors
- Reinforce inaccurate beliefs if a user phrases something in a convincing way because AI tends to provide responses that please the user
In psychotherapy, interpretation of reported symptoms matter, and AI is not always reliable in being accurate. A trained therapist doesn’t just provide information; they evaluate context, challenge distortions, recognize patterns, and adjust interventions based on your unique history, personality, culture, and goals.
Users may not be able to distinguish when AI is wrong, especially when the response sounds polished, compassionate, or authoritative. That can create a false sense of trust in information that may be incomplete, biased, or clinically inappropriate.
No Clinical Assessment
One of the most significant limitations of AI in mental health is that AI does not conduct a true clinical assessment. In good psychotherapy, treatment should not begin with simply offering advice or coping strategies. Two people can report the same symptoms, but the causes and maintaining factors can be vastly different. A trained mental health professional needs to gather critical information specific to each person so that appropriate treatment strategies can be chosen. An assessment may include:
- Current symptoms and their severity
- Duration and frequency of symptoms
- Medical history and possible physical contributors
- Trauma history
- Family and relationship dynamics
- Substance use
- Sleep, appetite, and energy changes
- Safety concerns, including risk of self-harm or suicide
- Patterns of thinking, coping, avoidance, and emotional regulation
- Differential diagnostic questions to rule out diagnoses with similar symptoms
Without a thorough assessment, it can be easy to misunderstand what is actually driving someone’s distress. For example, difficulty concentrating could stem from Attention-Deficit/Hyperactivity Disorder (ADHD), anxiety, trauma, sleep deprivation, depression, grief, burnout, a medical issue, or something else entirely. Racing thoughts might be related to stress or could point to something more complex, such as Bipolar Disorder, trauma, substance use, or another underlying condition.
AI may generate suggestions based only on the few details a person types into a chat. It cannot observe body language, hear tone changes, recognize incongruence, notice avoidance, or ask the nuanced follow-up questions that often lead to deeper clinical understanding. As a result, people may receive advice that feels helpful on the surface but misses the root cause of what they are experiencing. A concerning effect of this is it unintentionally delays people from seeking appropriate professional care.
In mental health treatment, the right intervention depends on the right assessment.
Counterproductive Reassurance
Mental health clinicians are increasingly noticing that people are using AI as a form of reassurance-seeking, using it for comfort, certainty, or relief when faced with uncertainty or emotional discomfort. The problem with this is that it can unintentionally reinforce the very patterns someone may be trying to overcome.
Many people seek reassurance to quickly feel better when they feel anxious, uncertain, guilty, or overwhelmed. In the moment, reassurance can feel comforting because it temporarily lowers distress or creates a sense of certainty. We all seek reassurance from others, and this can be adaptive by calming our nervous system and helping our minds to move onto something more productive. However, for some, this reassurance-seeking pattern causes significant distress or interferes with life, especially in relationships. Many mental health conditions, such as Obsessive-Compulsive Disorder (OCD), Generalized Anxiety Disorder (GAD), Illness Anxiety Disorder, and perfectionism involve repeated reassurance seeking; this often strengthens the anxiety cycle in the long term even though the person seeking reassurance feels better in the short term. Why? When our brains learn that a behavior meets our needs, it will encourage us to continue using that behavior in the future. For some, it becomes a significant problem that often needs mental health professionals.
Because AI is quickly responsive, always available, and often validating, it may repeatedly answer questions that reduce anxiety in the moment but unintentionally reinforce dependence on external reassurance. AI replaces the person providing the reassurance, and while this may help relationships, it does not help the struggling person feel better in the long term. They continue to chase reassurance because it works.
For example, someone with GAD may ask:
- “Do you think my husband is okay? He usually texts back by now.”
- “Can you tell me if I handled that conversation at work okay?”
- “Do you think my child will be safe on this school trip?”
- “Am I making the right decision, or should I rethink it one more time?”
After receiving reassurance, anxiety may temporarily drop. But the brain can quickly learn: When I feel uncertain, I need someone (or something outside of me) to make me feel better.
Over time, this can strengthen reassurance-seeking as a safety behavior. Safety behaviors are behaviors people do to reduce anxiety or prevent feared outcomes in the short term but often keep anxiety going in the long term. Reassurance-seeking can become one of those behaviors.
Examples might include:
- Repeatedly asking a partner, “Are you sure you’re not upset with me?”
- Checking with friends, “Do you think people liked me at that event?”
- Asking AI multiple versions of the same health question after noticing a body sensation
- Re-reading text messages, emails, or conversations to make sure nothing “went wrong”
- Asking AI or others, “Would a good parent have handled that differently?”
Each time reassurance reduces distress, the brain receives the message that uncertainty is risky and reinforces the idea that the person cannot cope should the feared outcome occur. Rather than building confidence, distress tolerance, or trust in one’s own judgment, the person may become increasingly reliant on reassurance to feel okay.
A trained therapist recognizes when reassurance is becoming part of the problem. Instead of repeatedly offering certainty, they may help a person:
- Tolerate uncertainty
- Reduce safety behaviors
- Build confidence in their own decision-making
- Sit with “not knowing” without immediately trying to eliminate discomfort
- Practice evidence-based interventions such as Cognitive Behavioral Therapy (CBT) or Exposure and Response Prevention (ERP) when clinically appropriate.
Sometimes what feels supportive in the moment can unintentionally keep someone stuck. Effective psychotherapy is not always about helping someone feel better right now but is often about helping them build the skills to feel better long term.
AI Can Sound Empathetic Without Truly Understanding You
One of the most powerful aspects of psychotherapy is not simply having someone listen but feeling deeply understood by another human being. As humans, we are wired for connection. Our brains and nervous systems are shaped through relationships, and healing often happens within strong relationships. When AI is used as a substitute for therapy, people may miss one of the core ingredients that makes psychotherapy effective: the experience of genuine human connection, attunement, and being understood by another person.
A trained therapist is not just listening to your words. They are observing:
- Facial expressions
- Changes in tone of voice
- Body language
- Hesitation
- Contradictions
- Emotional avoidance
- Patterns that unfold over time
AI can generate responses that sound compassionate, but it does not actually understand your pain, your history, your attachment patterns, your trauma responses, or the deeper meaning behind what you are saying. It predicts language but does not experience or provide human connection. It’s not human and cannot understand the human experience. Humans bond over shared experiences and feelings, and AI simply cannot provide this.
Human connection is not a bonus feature of therapy; it is often part of the treatment itself.
AI Often Tells You What Feels Good, Not What Helps You Grow
For those who’ve used AI, you know this. AI often tells you what it thinks you want to hear. It’s giving you a positive experience using it; not only does this make you feel good but it ensures that the AI company has happy users.
Psychotherapy is not always comfortable.
A good therapist may gently challenge:
- Cognitive distortions
- Avoidance patterns
- Self-sabotaging behaviors
- Maladaptive coping mechanisms
- Relationship dynamics that keep you stuck
AI systems, however, are often programmed to be agreeable and conversational. This can create a dangerous tendency for AI to validate whatever a user says even when the user’s thinking is inaccurate, distorted, or harmful.
There are concerns about AI “sycophancy,” where AI reinforces users’ beliefs rather than providing accurate and unbiased information. In mental health, that’s very concerning.
AI Cannot Reliably Assess Risk or Manage Crisis Situations
A licensed therapist is trained to recognize warning signs of:
- Suicide risk
- Self-harm
- Domestic violence
- Abuse
- Psychosis
- Substance misuse
- Severe depression or mania
These situations require nuanced clinical judgment, ethical decision-making, and sometimes immediate intervention.
AI does not hold a license, cannot complete a formal risk assessment, cannot coordinate emergency care, cannot contact supports, and cannot help to ensure safety.
Privacy Is Not Always What You Think
With psychotherapy, licensed clinicians are bound by professional ethics, confidentiality laws, and privacy standards. Many AI tools are not held to the same standards.
The American Psychological Association (APA) has warned consumers not to rely on AI for psychotherapy or psychological treatment due to concerns about privacy, safety, and lack of evidence-based oversight.
Before disclosing deeply personal information to a chatbot, people should ask:
- Who has access to this data?
- How is it stored?
- How might it be used in the future?
Can AI Have a Place in Mental Health?
AI may eventually support mental health by helping with:
- Psychoeducation
- Mood tracking
- Journaling prompts
- Practice between sessions
- Access to general coping strategies
But a tool is not the same as treatment. General education is not the same as professional treatment.
Now What?
We live in a world that increasingly values speed, convenience, and instant answers, but healing rarely works that way. Psychotherapy is not designed to simply provide reassurance or validation because this does not actually help long term. Psychotherapy is a collaborative process in which a trained clinician helps individuals gain insight, confront maladaptive patterns, develop healthier coping strategies, and better understand their thoughts, emotions, and behaviors so that they can live the life they want in accordance with their values.
Just because AI can simulate conversation does not mean it cannot replace human connection, clinical judgment, ethical responsibility, or the transformative power of a genuine therapeutic relationship.
